North Memorial Health Hospice

As a nationally recognized pioneer, North Memorial Health Hospice has been leading the way for over 25 years with an abiding commitment to helping individuals live every day fully and completely throughout the final states of life’s journey.

At North Memorial Health Hospice, we care for both patients and their families by providing services wherever an individual resides:

  • At home
  • In a skilled nursing or assisted living facility
  • On the Palliative Care Inpatient Unit at North Memorial Medical Center
  • At the North Memorial Residential Hospice (or other hospice residences)

Relief of a patient’s physical symptoms is our first priority. That is why the most advanced symptom control and pain management methods are used. Yet, hospice care goes far beyond simply meeting one’s physical needs. It also involves emotional and spiritual support for patients and families. Patients are encouraged to live each day to the fullest and to embrace the love of family and friends.

At Home Care

Home is the place that is so essential and important all of our lives. Even when faced with serious illness, hospice patients can be in the comfort of their own home. The North Memorial Hospice Home Care staff provides support, knowledge, teaching and care to those who desire to live at home or with their family.

Skilled Nursing and Assisted Living Facilities

Our Hospice Home Care staff provides visits and brings symptom management expertise to patients in the skilled nursing and assisted living facilities. North Memorial Hospice staff work hand-in-hand with the facility staff, sharing the goal of providing physical, emotional and spiritual support for the residents and their families.

Inpatient Palliative Care Unit

Working with your physician, the Inpatient Palliative Care Unit staff addresses issues such as the adjustment to changing physical conditions, medication needs, symptom control and care requirements. Patients and their families will experience a calm and reassuring atmosphere, a welcoming environment and experienced staff with knowledge of what can increase comfort.

Residential Hospice

This eight-bed Residential Hospice is located on two acres of lake property on Upper Twin Lake, in Brooklyn Center. It is a welcoming and inviting home in the community for patients who need 24-hour care, are no longer able to care for themselves, have no family to provide the care, or prefer a home-like atmosphere outside of their own residence. The eight private rooms all have a view of the lake, a private bathroom and are equipped with a sofa sleeper for family members’ comfort. The skilled are provided at the residence allows family members to focus on the role of being a family member.
