The Sholom Johnson Hospice philosophy recognizes no matter what your faith, every person deserves to live out his or her life with respect and dignity, free from pain, and in an environment that promotes quality of life.

Sholom’s Medicare-Certified Hospice fulfills a crucial need in the Twin Cities. The Hospice program combines the traditional Jewish values of acts of loving kindness, human dignity, and community with modern tools to care for those individuals facing death, as well as their families.

Sholom Johnson Hospice serves patients from all faiths and cultures. Our staff is trained to meet the specialized needs of Jewish patients and their families.

Hospice services are delivered wherever the patient calls home: a private residence or apartment, a nursing home or assisted living residence. Hospice suites are available on Sholom campuses as well. A highly trained interdisciplinary team partners with the patient, family, friends and the primary physician to create an individualized plan of care.

Hospice services are covered through many insurance plans including Medicare and Medical Assistance, as well as private pay.

It is the policy and commitment of Sholom that it does not discriminate on the basis of race, age, color, sex, national origin, disability, religion or political belief.
